Parental Authorization: You agree that your child has permission to participate in the above program, and you will not hold the coaches, camp, nor sponsors responsible for injuries that may occur while participating. There is a $50 non-refundable deposit for each camper.

Photo Consent Waiver: Please read and select your response below. Stuart Malcolm Soccer Camp has my permission to use my or my child’s photograph publicly to promote the camp. I understand that the images may be used in print publications, online publications, websites, and social media. I understand no royalty fee or other compensation shall become payable to me by reason of such use.
